Care Navigation is a free service designed to walk alongside individuals and families who are facing serious illness but aren’t yet mentally or emotionally ready for hospice care. It offers a gentle, supportive way to explore care options, understand what’s ahead, and begin making informed decisions—at your own pace.
A nurse will stay connected through regular phone check-ins, answering your questions, listening to your concerns, and gently supporting you as you explore the benefits of hospice care.
You don’t have to go through this alone. Care Navigation helps you feel supported, informed, and cared for—no matter where you are on the journey.
